STEP 1:
Registration – Middle School Sports Registration
- Click the Middle School Sports Registration
- Fill out form completely
- Submit Form
STEP 2: Sports Physical
- Sports Physical Form : English
- Sports Physical Form : Spanish
- Medical Physician must complete the form
- Bring completed form to Kori at Taft 7-12
- If your student has had a physical in the past two years, paperwork will be on file with Taft 7-12 Athletic Secretary
STEP 3: Athletic Fees
- $100.00 Fee for Middle School Sports
- Students can pay online (CLICK HERE) or come to the Taft office to pay fees.
Final Step: Practice
- Coaches will communicate with athletes about start date and practice times.
- Academic eligibility and regular attendance must be maintained during the season in order to participate.
