Taft Athletic Clearance 2022-2023

Remember to:

  1. Fill out Sports Physical Form
  2. Provide Proof of Insurance
  3. Register Athlete using Online form
  4. Complete Impact Test

*Turn in all paper copies to Kori in August

  • Current in the past 2 years.
    • Make a dr. appointment for a sports physical
    • Print form
    • Fill out form
    • Take form with you to the dr.
    • Dr. completes/signs  form
  • Bring to Kori at  Taft HS during clearance dates/times
  • Current physicals are on file with Kori
Proof Of Insurance
    • If your child needs to purchase insurance, you can purchase from this link:

  • If you do NOT need to purchase insurance, fill out your insurance info on the participation form below.
    • If your student purchased insurance
      • Print off this proof of insurance
  • Bring to Kori at  Taft High School during clearance dates/times

  • COVID Waiver is embedded in Fall Registration Packet

  • All Sections are embedded in the Registration Packet. These sections will be submitted electronically for the 2021-2022 sports seasons.
  • Email Impact results to kori.bayer@lincoln.k12.or.us 
Participation Fee  
  • ALL FEES WAIVED FOR 1 SPORT PER SEASON , PER ATHLETE Through a generous grant provided to LCSD, all participation fees are waived for one sport per season!
 If you play two sports in one season you will pay $125.  Ex. Cross Country and Soccer

  • Could be Emailed or printed
  • ALL ATHLETES:
    • Bring any completed items listed above in Orange to Kori  at Taft High School during clearance dates/times
  • Get grade check from Kori
  • Kori will give you a clearance slip to give to your coach.
