Medication at School.

Information for Students and Families...

If you need to take medication during the school day, all medications must be authorized through the
Health Office before they can be brought to or taken at school.

1. Medication kept in the Health Office
If your medication will be kept in the Health Office and administered by school personnel, the following are
required:
● Your parent/guardian must complete the HE-26 Authorization for Medication Administration form.
● Your parent/guardian must bring the medication and complete the medication check-in with Health
Office staff in person.
● Medication must be in its original, labeled, unexpired container. Prescription medication must have the
pharmacy label and written directions.
● The medication will be counted and documented by Health Office staff and your parent/guardian when
it is brought to school.
Do not bring medication to school before completing this process.

2. Medication carried and taken by the student
Students who are developmentally and/or behaviorally able may be approved to carry and self-
administer their own medication. This can include medications such as inhalers, EpiPens, prescription
medications, or appropriate over-the-counter medications.
● A HE-28 Authorization for Self-Medication Administration at School must be completed and approved.
● The HE-28 may be taken home to complete and then returned, with all required signatures, to the
Health Office.
● Medication must remain in its appropriately labeled original container with the student's name.
Prescription medication must be in a pharmacy-labeled container with only the amount needed for the
school day; over-the-counter medication should be in its original container with enough for one school
day.
● Students may not share or lend medication to another student.
● Self-administration of controlled substances is not permitted under the HE-28 procedure.
Questions?

If you are unsure which form you need or have questions about bringing medication to school, please
check with the Health Office.
